Dear Member

In an effort to better understand some of the factors that may affect the lives of individuals with Crohn’s or ulcerative colitis, Donna Bacchus, a PhD candidate at the University of Texas at Arlington, is conducting a research study. It will take you approximately 10-15 minutes to complete this anonymous survey. Your input is very important, and will be strictly confidential, used only for the purposes of research for this project. If you are between 18- 65 years-old, currently on medications, and have Crohn’s or ulcerative colitis, your input can help with improving medication adherence. It will also help to understand difficulties that patients have with taking medications. Please note that the information you provide will not be linked to your email, name, or any identifying information about you.

If you have any questions regarding this study, or would like additional information to assist you in reaching a decision about doing this study, please contact me at phone number (1-469-999-1356) or by email [email protected]

You may also contact my supervisor, Dr. Jennifer Gray by email [email protected]

Simply click on the link below to access the survey:
